    7. If tree replacement is required, please provide the number of trees, common name of species, and size specifications. Planting location is to be provided on a site plan.
    8. A Certificate of Occupancy for new construction may be withheld to ensure completion of a tree planting program.
    9. Authorization
      This application must be signed by both owner and applicant when the owner will be contracting any part of the work out to a second party. Both parties shall be responsible to ensure that tree removal and replacement planting as proposed herein are properly performed and completed.
    10. Certification
      I hereby certify that I have read the tree removal permit requirements and, to the best of my knowledge and belief, all information supplied in this application and supporting documents is true and accurate.
    11. Note: Both of the above lines must be signed, even if they are one and the same person.
